All applicants seeking to drive a commercial motor vehicle as defined by 49 C.F.R. 383.5 should start with their most recent job, include active military assignments and voluntary employment and provide ten (10) years of history. (A separate sheet may be attached.) You must explain any gaps in your employment history. 49 C.F.R. 383.5 defines commercial motor vehicle as a motor vehicle or combination of motor vehicles used in commerce to transport passengers or property if the motor vehicle
(a) Has a gross combination weight rating 26,001 pounds or more inclusive of a towed unit(s) with a gross vehicle weight rating of more than 10,000 pounds; or
(b) Has a gross vehicle weight rating of 26,001 pounds or more; or
(c) Is designed to transport 16 or more passengers, including the driver; or
(d) Is of any size and is used in the transportation of hazardous materials as defined in this section.
All applicants seeking to drive a commercial motor vehicle as defined by 49 C.F.R. 390.5 should start with their most recent job, include active military assignments and voluntary employment and provide three (3) years of history. (A separate sheet may be attached.) You must explain any gaps in your employment history. 49 C.F.R. 390.5 defines commercial motor vehicle as any self-propelled or towed motor vehicle used on a highway in interstate commerce to transport passengers or property when the vehicle
(a) Has a gross vehicle weight rating or gross vehicle weight or gross combination weight of 10,000 pounds or more, whichever is greater; or
(b) Is designed or used to transport more than 8 passengers (including the driver) for compensation; or
(c) Is designed or used to transport more than 15 passengers, including the driver, and is not used to transport passengers for compensation; or
(4) Is used in transporting material found by the Secretary of Transportation to be hazardous under 49 U.S.C. 5103 and transported in a quantity requiring placarding under regulations prescribed by the Secretary under 49 CFR, subtitle B, chapter I, subchapter C.
Note to Applicants: (Part 391.21 (d)) The information provided in this section may be used, and prior employers may be contacted, for the purpose of investigating the applicant’s background as required by FMCSR Part 391.23. I understand that employment at Kimball Fuel is “at will,” which means that either I or Kimball Fuel can terminate the employment relationship at any time, with or without prior notice, and for any reason not prohibited by statute. All employment is continued on that basis. I understand that no supervisor, manager, or executive of Kimball Fuel, other than the president, has any authority to alter the foregoing. I further acknowledge that, prior to starting employment, I will be provided with several company policies and an Employee Handbook which shall not be construed as a contract and are subject to unilateral change by Kimball Fuel.
